Accidental Death And   Dismemberment Plan

General Description of Coverage

The Accidental Death and Dismemberment (AD&D) Plan is available to all employees and their dependents who are eligible for health benefits with the University.  It is offered through METLIFE.  The Plan provides benefits in the event of an accidental death or dismemberment.  You can choose either Employee Coverage, or Employee and Family Coverage.  The plan offers three different levels of coverage:

Medical Review:
No medical review is required for enrollment in the Plan.  This Plan will cover you for accidents that occur at work as well as accidents off the job.

Additional Benefits:
The PA&D Plan has many other benefits in the event of a covered loss. Please contact your Benefits Services Counselor to review the plan booklet containing a detailed description of the additional benefits.  Additional benefits include:

  • Exposure and Disappearance

  • Double Indemnity for Dismemberment and Paralysis

  • Home Alteration and Vehicle Modification

  • Waiver of Premium

  • Education

  • Day Care

  • Seat Belt

  • Common Disaster

  • Coma

  • Emergency Evacuation

  • Repatriation of Remains

Exclusions:
The Plan does not cover any losses caused by or resulting from: (1) self-inflicted injuries; attempted suicide; or suicide; (2) war; (3) full-time active duty in the armed forces of any country or international authority; (4) flying as a pilot or crew member on any aircraft; flying as a passenger (or otherwise) in any aircraft used for tests or experiments; or (5) any Military Aircraft except for aircraft operated by MAC or similar service of another country.

How to Receive PA&D Plan Benefits

Benefits will be paid to the Insured Person within 365 days of the date of an accident.  The Plan will pay, in one sum, a certain percentage of the Principal Benefit Amount, depending on whether there is a loss of the life or some type of dismemberment.  If more than one covered loss is sustained during one accident, the Plan will pay all losses up to the Principal Sum Amount.
